ADDING A PLAYER TO A TEAM – Add or Transfer
Whenever a player is added to a team, you need to include the following:
- state registration form – you must include club #; team # (same as roster#), pass ID #
- medical release – needs to be notarized
- birth certificate or passport (copy only!)
- new card- completed on front, signed on back w/ picture of player
- completed status form (indicating if they are: 1) an add - meaning they have not been carded to another team during the seasonal year -fall or spring; or 2) a transfer - meaning they were on another team and are transferring to yours - a copy of the release from the old team must accompany the transfer)
- If the player is a transfer, be sure to attach a photo copy of his release from prior team for your DC as proof the player was properly released from his/her prior team.
REMEMBER – can only fill a maximum of 21 total roster spots for full sided (17 small)-w/ 18 active players at any time-to the roster over the course of a seasonal year, with a maximum of 3 transfers (from other teams). For U17 & older teams, the maximum is 22 roster spots and all can be active; with only 18 players brought to games.
RELEASING PLAYER FROM A TEAM ROSTER
When you take a player off of a team, you need:
- Status form completed under release & top portion – This form MUST be signed by player. If they have moved and can not sign, please include note on top of form with their new address. Better to get them to sign release BEFORE they move.
- Their player pass from the team they are leaving (MUST BE TURNED IN TO DC who signed the first pass) must be attached to release.
- The DC who signed the pass of the player requesting to be released MUST be presented with the release form for signature. ANOTHER DC SHOULD NOT SIGN FOR THE ORIGINATING DC, unless the DC’s agree and the originating DC gives their permission for the transferring DC to sign on their behalf.
- A player MUST be released from the team before he/she can be added to a new team
When rostering a new team:
- Coaches MUST complete a volunteer state registration form & attach a copy of their license
- If the coach is carded to another team, list the card number on the new roster, and complete another state registration form for the second team – use the pass number assigned from first team.
- If the coach is a non-volunteer coach, he/she must complete the non-volunteer state registration form & go directly to the state office to have a pass issued.
- If the non-volunteer coach is already carded, he/she should complete the non-volunteer state registration form, and include his/her pass number on the form and then they shall be added to the roster.
Must include for each player:

ADDING A COACH TO A TEAM -
When you add a VOLUNTEER coach to a team, you will need to include:
- state registration form - 2 copies – volunteer (this will mean the pass from packet will be used)
- copy of coach license
- new coach card, completed on front & signed on back w/ picture (volunteer) ready to be stamped by DC – photocopy state issued pass for non-volunteer coaches
- Status form completed and signed as new coach (ALL COACHES)
NON-VOLUNTEER COACHES CAN ONLY BE CARDED THROUGH THE STATE OFFICE
$200 fee if no insurance is presented; $30 processing fee for those who do present insurance certificates
After card is received through the state office, complete a status form, and include a copy of the state issued pass and completed status form to add non-volunteer coach
